What UHL says about discharge day

The current University Hospital Limerick patient booklet says the medical team or ward nurses confirm discharge. A patient may leave from the ward or be transferred to the Hospital Discharge Suite before leaving.

The booklet describes the suite as an area where final medication or treatment, prescriptions, discharge letters and transport arrangements can be completed or followed up. It says the suite is run by nursing staff and is open from 8am to 6pm, Monday to Friday. Confirm current arrangements with the ward because operational details can change.

A companion can carry discharge documents, prescriptions and belongings. They do not interpret clinical instructions.

Distance, vehicle access, mobility needs and availability must be confirmed before relying on a booking.

Practical settling in

The agreed scope may include keys, belongings and a family update. Nursing, personal care and clinical monitoring are excluded.

Cases that need a different transport plan

A private companion is not an ambulance or clinical transport service. Tell the ward about any mobility, oxygen, infection-control, behavioural, clinical-observation or equipment need. The hospital team decides whether ordinary transport is appropriate.
