We work with you, not around you

Our companions check in at the ward, coordinate with nurses on medication and instructions, and follow your discharge plan. We are an extension of your discharge process, not a replacement for it.

How a referral works in practice

1. You identify a patient

A patient is medically fit for discharge but has no safe way home. You've checked HSE Patient Transport and exhausted the obvious family options.

2. You hand a leaflet to the family

Or, if the patient has no family available, you ring us directly on the hospital line and we take it from there.

3. We confirm within the hour

We confirm collection time, companion name, vehicle, and contact number. You'll get an email confirmation if you want it on file.

4. The companion arrives

They check in at the nurses' station, identify themselves, collect prescriptions and instructions, and walk the patient out to the car.

5. Safely home, you're notified

We text or email the discharge office when the patient is safely home and settled. Bed freed up, patient safe, family informed.

6. We follow up if appropriate

For Premium bookings we follow up with the patient 24 hours later and flag anything concerning back to your discharge team if relevant.
